1. CLICK HERE to download a BLANK copy of the Black Star Fan Gear Order Form.
2. Fill out the form completely and DOUBLE CHECK all spelling, sizes, and numbers (if applicable).
3. Scan  the completed document to your computer or phone.
3. Download  and Attach one or more of the following full color images to your order:

WOMEN'S SHORTS

4. Email your order form and apparel images to Brent & Alyssa at Black Star Athletics.
Be sure to include the email address of the individual responsible for paying invoices.
