Uniform Ordering

FOR EXISTING TEAMS, COACHES, AND PLAYERS ONLY

PLEASE USE ONE OF THE FOLLOWING OPTIONS TO ORDER UNIFORMS FROM  BLACKSTAR:

OPTION 2:
1. CLICK HERE to download a BLANK copy of the Black Star Uniform Ordering Form.
2. Fill out the form completely and DOUBLE CHECK all spelling, sizes, and numbers.
3. Scan  the completed document to your computer or phone.
3. Download  and Attach one or more of the following full color images to your order:
A. BLUE PINSTRIPE JERSEY w/ BLUE PINSTRIPE PANTS
B. RED PINSTRIPE JERSEY w/ WHITE PANTS
C. SOLID BLACK JERSEY w/ BLACK PANTS
D. SOLID WHITE JERSEY w/ WHITE PANTS
4. Email your team's completed order form and uniform images to Brent & Alyssa at Black Star Athletics.
Be sure to include the email address of the coach/treasurer responsible for sending payments.

5. Pay the invoice sent via QuickBooks link in the email from Black Star.
(Note: Be cautious of fraudulent emails requesting payments - Always check the email address to confirm it is legitimate)
